Health Screenings

The school nurse conducts health screenings required by law. The Texas Dept.of State Health Services requires vision, hearing and spinal screenings. All screenings are completed by school nurses and certified staff.  If any problems are found, they are reported to the parents and a referral to a health provider will be made for further evaluation.  

Screening Schedule

Vision Screening: PreK, Kindergarten, 1st, 3rd 5th and 7th graders

Hearing Screening: PreK, Kindergarten, 1st, 3rd, 5th and 7th graders

Spinal Screening: Girls in 5th and 7th grade and Boys in 8th grade.

What is required to attend school?

  • Immunizations – up to date on immunizations, including Tdap and MCV for incoming 7th graders.

  • Physical Exam -  7th graders participating in sports need a UIL physical.

 

Medication Policy: 

  • OTC or Physician/Parent Prescription form with physician and parent/guardian signature needs to be on file.

  • Original container, with pharmacy label if a prescription

  • OTC medication, i.e. tylenol/ibuprofen must be in original container, age appropriate, and within date.  

  • Parent must drop off medication. Children are not allowed to deliver medications.

Communicable Diseases:

  • A doctor’s note is required when your child has been absent with such illnesses as strep throat, scarlet fever or questionable rashes.  If you have any questions, please keep in touch with me.

 

 SICK DAY GUIDELINES:

  • Fever over 100.4 degrees

  • Undiagnosed rash

  • Vomiting/diarrhea

  • Red, watery eye with yellow discharge

  • Symptoms that prevent them from participating in school, i.e. uncontrollable nasal discharge, excessive coughing or fatigue, sore throat.

  • Please keep your child home for 24 hours after vomiting or diarrhea has stopped.

  • Student’s should be fever free for 24 hours without fever-reducing medication before returning to school.

 

 Food Allergies Can Be Life-Threatening!

  • A physician note/parent note must be on file for all food allergies.  

  • A Physician/Parent Prescription form needs to be on file for all EPI-Pen orders.

Please help the school nurse by:

  • Updating the phone numbers on your child’s school record so the nurse can always reach you

  • Sharing if there is an event at home or in the family that may impact your child at school such as military deployment, birth, or death of a family member or of a beloved pet.
